Day 10 in PCICU - Day after Extubation

Christina (16) is waiting for her little sister to get back home for all of those sister moments!



As I had posted last night, we needed to get through last night to sort of "weigh" if she could handle the step of extubation and remain off of the ventilator. I hadn't posted anything yet today, because I sort of felt like we needed today too as she only made a very small amount of progress since last night.

I will say however, at rounds, the Dr said she is doing well and is making progress! I feel confident they will not need to reintubate. I am trusting in God for that.

Here is the progress she is making:

Her oxygen saturation levels are good - mid 90's.

Her heart rate is good.

She is coughing up the junk.

She is calling "mom" in a hoarse voice 500 times an hour. :)

Blood gas levels are good.

She said, "I'm hungry" and ate a whole popsicle and some chicken broth.


Here is where she needs improvement and specific prayer:

1) Her blood pressure is still high. They tried to dial back down the BP med today just a bit but her pressure got way too high so they had to go back up.

2) She still has that effusion in her lung. It is smaller but still there. (they do an x-ray every morning). Dr believes she has some Pneumonia. :(

3) She is VERY weak. Cannot lift her head. Cannot interact or focus yet.

4) She is having terrible withdrawals from the drugs. Shaking, jittering, crying, fever.

A QUICK UPDATE TONIGHT FOR THOSE WONDERING

Hua Hua is stable tonight. Praise God she is resting and breathing.

They gave her medicine for her blood pressure and that came down. It is still high, but not extreme.

They switched her to a different canulla (the nose oxygen blower thingy) that has a higher rate of oxygen combined with humidity. Her breathing is still fast, but fine and her Saturation levels are awesome.

She has been very tired, and has kept her eyes closed even though awake often. She feels very yucky and has had a lot of pain because she cries silently a lot.

She needs to rest tonight and adjust and gain strength. Tonight is the night for her to make it through without incident. If she can, I feel certain they will not have to re intubate again.

Sweetness free from the breathing tube this morning



Katie Grace Hua Hua was extubated this morning at 9:15. She did well for the first hour, but she was breathing VERY fast the whole time.

She was also not very awake, but somewhat awake.

She was dealing with a TON of very sticky goo that was in her throat that kept coming up into her mouth and also gagging her. A lot of it developed from the other day when she extubated herself and they had to reintubate and try many times irritating her throat. This incident also has caused her throat to swell. All that goo was rattling.

She also was taken off 3 drips over the past 12 hours. She was clearly dealing with some withdrawels, but wasn't awake enough to take the methadone by mouth to help come down.

Breathing very fast caused her heart rate to soar, which caused her pressure to rise.

Feeling sick from coming off meds and all that goo made her throw up a gallon of bile.

She started to really struggle and they all came running. She wasn't breathing well and pressure was bursting.

She was and is VERY weak.

They cleaned her up and changed her canulla to a higher dose of oxygen w/humidity. She has a little morphine for relief and she is stable.

THE HAVE TOLD ME THAT THEY ARE NOT SURE IF THE WILL OR WILL NOT HAVE TO REINTUBATE.

They will watch her for the next 12 hours and see how she recovers. She is really struggling and needs to gain strength!

THE WEAN IS ON

Before I tell you her stats, let me tell you the sweetest thing! I brought her favorite pink blanky from home. The one Lolli made her that is silky on one side and satin on the other. Well, she woke up a few minutes ago, (oh and by the way, whenever I have said "woke up" that basically means moving with eyes closed or maybe open slightly for a brief flutter), and I put her blanket on her and said here is your blanky from home, it's your favorite, isn't it? And she nodded her head! That is the only time I have received a certain response from her as of yet during this. She really does hear a lot more than we think! Oh my heart.

Just a quick update tonight in case anyone is awake and checking in. Of course I am awake and should be sleeping.

I want to let you know that Katie Grace Hua Hua is being weaned of her sedatives tonight little by little. She should be off of most by 6AM. Then they will be prepared to attempt to extubate her sometime after that. They want to make sure she is awake and able to cry and get the gunk out when they do.

They have the dial down to 12 breaths per minute on the oxygen now, she is breathing at around 25 breaths per minute, so she is breathing over half now, which indicates she is ready to take over.

She is at 65% oxygen. They will leave it until extubation and then switch her to a canulla nose feed when she is awake.

Pray all goes well, it can sometime be a dramatic change as they stop all the drips and take them off after being on for so many days. Pray this really happens! Thank you for your prayers! So many people everywhere, praying for this little girl. Thank you.

WHEN THE MELTIES MELT AWAY AND A RAY OF LIGHT SHINES -PCICU Day 8




Joshua sure misses his sister and playmate. They are stuck like glue at home.

In spite of the craziness and all the melties yesterday, Hua Hua recovered quite well. Thanks to your prayers and the mercy of our Lord.

As of last night the Dr starting talking... extubate. Yes, I said it, EXTUBATE! He said she recover from trying moments well and her numbers continue to look good.

Right now her Oxygen Saturation level in her blood is at 94, which is great and if she continues to hold Saturation in the 90's, along with healthy pressure they have made a plan to extubate. Their goal is: tomorrow noon! Yes, seriously, she may be coming off the ventilator tomorrow!

She will have to be successful to meet these goals, and stay off too.

PLEASE PRAY because there are a lot of factors that have to be successful for the extubation and this is stressful to her body:

1) She is off Nitric Oxide completely as of this AM!

2) Her Oxygen assistance in the ventilator machine is giving her 80%, they will wean it by 10s every 4 hours starting now.

3) Her Oxygen BPM (breaths per minute) assistance with the ventilator is 20. She will be weaned down by 2 every 4 hours until they get to 10 so that she will have to breath herself in between and then they will just cut when they are ready.

4) Her sedation and pain medications are heavy and addictive. They will start weaning from several of them, bit by bit starting this afternoon. They make her breathing weak and she depends on the machine to breath, so she has to come down off a few.

5) She needs to have a BM. Sorry to be so frank, but they have been feeding her though a tube and she hasn't had one, and they really like to see one! Who wouldn't!? :)

I will let you know how she is doing tomorrow!


Here are prayers answered from yesterday!:

Her central line was fixed and they started her on a new strong sedative. I'm not sure I am thrilled about more drugs, but she is sleeping and pain free. They have planned her changeover to lighter doses for weaning.

Her fever has been down for over 12 hours. Thank you!

Like I said, she tolerated being taken off the Nitric Oxide, yay!

MELTY, MELTY, MELTY - We had an Emergency - Day 7 Post -Op


Just to lighten the mood, I thought I'd post this pic of Hua Hua after she colored on her face with a marker. Just look at her face after I said, "Hua Hua! What did you do??" Oh we laughed about this!




Ok, so remember yesterday when I talked about that feeling I get when I am SO scared that I tremble inside and my stomach and legs feel all MELTY?

Well, THE MELTIES struck again.

BIG TIME

Let me first say that Katie Grace Hua Hua is okay this morning and her numbers are good. So that we are so thankful for.

Now, back to THE MELTIES.

We had 3 MELTY EVENTS. 2 durning the night and the GRAND FINALY of MELTIES this morning.

At around 1:30 AM, Hua Hua woke up and seemed to be in pain. She grimaced and made a cry face. (she can't actually cry with all her tubes). So the nurse gave her morphine and adivan. Then she took her temp and it was 102.3F (I found out later it was 103.7F :( an hour later). Then Hua Hua began to twitch and jitter.

THE MELTIES began.

Well I had not seen her do this yet, in this setting or ever. I told the nurse to come and look at her. We weren't sure what it was. I suggested she was having a reaction to the meds. She thought the fever. Well, it wasn't shivering and she didn't have goosebumbs on her arms like you do when you get the chills w/fever, she was jerking. So the nurse woke up the Dr and he came up and said it was the fever. They gave her Tylenol and started back on antibiotics.

I fell back asleep and awhile later the dingers go off and Respitory comes in to suction her vent tube. She notices that there is a lot of goo in Hua's chest, so she has to squirt saline down the breathing tube, push the line down and suction the goo back up. She did this 3 times. This was awful for Hua Hua.

Again came THE MELTIES

SOOO... at 5:30 AM, her dingers go off, and I am up on the 1st ding. I look at her and see she is moving her mouth a lot and there are bubbles coming out. I ask someone walking by to get my nurse quick and the tech looks in and sees that the Hua Hua has extubated herself!?? Well my heart just dropped to the floor. She has too much sedation to breath for herself!!!

BIGGEST CASE OF THE MELTIES EVER

Every Dr and nurse on the floor rushed to her room. They began breathing for her with a hand bag and for the next 40 minutes tried to reintubate her, while she was still awake :(. They tried twice but not successfully. So they called in the anesthesiologist from the ER and she got it in. Then they had a hard time getting one of her nose tubes in. Meanwhile they are still using the hand bag ventilator. They were successful and she did not ever lose too much oxygen.

TOOK QUITE AWHILE FOR THOSE MELTIES TO MELT AWAY

Yesterday's prayers answered:

She wakes up less and less fearful, although there are still some sad moments, she is more like a dazed wiggle worm the past few times. Thank you for covering her in prayer for this.

New medication is working well, she is going to be of N.O. today hopefully!

Albuterol is effective. The Dr said to not worry that it makes her heart rate go way up for that time.

Please pray for today:

1)Her Medications for pain and sedation:
They believe that either she moved her central line out enough to have her not absorb all of her medication today or she is getting used to it. But each time she got medication today, she has started twitching, jumpy, shaky and waking 30 min later. Like she is coming off something. Please pray they they decide the best avenue for her.

2)Her Fever:
Pray the find the source of her fever. They have her on antibiotics again which is awesome. One theory is that her breathing tube could have gotten a little infected. Well since she pulled it out this morning, maybe it was meant to be.

3)Her numbers are doing well last night and today. They are thinking they might be able to extubate by Friday. Pray she doesn't have any setbacks and can wean off everything. She is down to 4% Nitric Oxide! Just 3 to go. She is still at 90% oxygen and will be taken down by 10s to 40%.

THANK YOU FOR YOUR PRAYERS! IF YOU WOULD BE SO KIND TO PRAY THIS TODAY - DAY 6 POST-OP


Do you ever get that feeling of being so scared that you tremble inside and your legs and stomach feel melty?

That is the feeling I get on many occasions over the past few days. Don't worry. I don't have it constantly. Sometimes I have great hope and courage. Many times I am just sitting here reading or on the computer and absolutely nothing is happening. But then, it sweeps through...

Katie Grace Hua Hua will wake up and be so agitated and try to get up or put her hands toward her face (to pull at those tubes) even thought her hands are tied to the sides of the bed, she will kick her little legs in anger, she will look at me with fear in her eyes, and then she will start to try and expel the tube in her throat and her face will turn red, and her body will convulse as she coughs with no sound, and all her stats will go up when they should be down, and down when they should be up, and the dingers will go off.

I try to calm her.
I get that feeling.

Then the nurses will go ask the Dr. if they can give her more sedation because they are in awe when they discuss how much she is on and that she would even be able to awake under such sedation, and I think, you don't know who this little, powerful, scared girl is who would claw her way out of the deepest sedation to get to her mama, and they will get permission, and fill her tiny body with more drugs.

I stand by and watch.
I get that feeling.

I will see her stats are good and her Oxygen stats are at 93 or higher and I will feel joy. Then I will wake up from a nap or come back from a lunch and see her stats at 89 and know she will not get closer to that tube out today.

I will the number up, with all my might, but it doesn't do what I wish.
I get that feeling.



Katie Grace Hua Hua is up and down as each day passes. Today, the Oxygen fed to her is at 90%. It needs to be at 30%-40% for her to take over and in order for them to take her off the vent. They will not be lowering it today, but will tomorrow.

For those of you dear friends praying, let me talk about yesterday's prayers:

The liquid in her lungs was blood mixed with just fluid. It is not Pneumonia! It was most likely due to a small bleed from a chest line that came out and bleed a little on the inside at the opening. The lung has been drained and the Xray is clear! Thank you!!

If you will pray for today:

1)Calm Her Fears:
She is waking up less because she is so sedated, but I would like and the Dr would like too that they will not have to use more sedation and if she does wake up , somehow we can calm her and let her know she is okay. How to do that with a 4 year old that doesn't understand what is going on? I don't know, BUT GOD DOES, and she doesn't need all of these heavy sedatives to this extreme.

2)New Medication is Working -Let's Get Her Off The Nitric Oxide:
The medication they are trying in the drip seems to be lowering her Pulmonary Hypertension pressure. They are trying to get her off the Nitric Oxide throughout today and tonight. That is half the battle with the vent. It is at 10 and we need it at 0.

3)Albuterol is effective, pray for her heart rate:
They gave her some Albuterol in her vent today because she had some "rattling" in her chest. It was very effective (she has used it at home) to open up her airways and brought her Oxygen Sats up to 98%! But caused her heart rate to go up to 160-180 for about 40 min, yikes. Very high and she was jumping out of her bed. Pray she can handle that. She needs those high Sats, but not high heart rate.

DAY 5 IN HOSPITAL UPDATE


I wish, with all my heart, I could say the Hua Hua was doing wonderfully and that she was off the respirator and in my arms, but that is not the case. She is still sedated and intubated and will be for some time.

For those of you who are following along and praying I will give some specifics and ask you to pray with all of your hearts.

A couple of concerns for PRAYER:

1)Liquid in Lungs:
Everyday they take an X-ray of her lungs in the AM. Katie Grace Hua Hua has had a light shadow in her right lung and it is growing. They have her on antibiotics, so if it is pneumonia, they are working on it already. They believe it could be or it is just some gooey junk collecting that gets by the breathing tube and is not get sucked out when the suck it out. They are going to palpitate her lungs today by thumping hard on her on the chest to get it moving. I doubt that will be comfortable after having her chest wired shut. :( Think of how tender broken bones are? They are also going to do a sonogram to look at it and see if they can put at chest tube back in and into the lung and drain the stuff out. This should help her OXYGEN SATS (the number we watch continuously).

2)Calm Her Fears:
Also, even thought they have her on many sedation drugs, she continues to wake up and try to get up. I knew she would do that. This child has abandonment fears. She will claw her way out of the deepest sedation to get to mom. She PANICS. When she does get upset and agitated, OXYGEN SATS go down. They take awhile to go back up.

3)Trying Another Medication in Drip to get her off Ventilator:
Yesterday afternoon and into evening, her SATS lowered a little, so they dialed Oxygen back up to 85% and Nitric Oxide back up to 20%. Not the direction she needs to go in, in order to get off of the ventilator. So they are trying a Pulmonary Hypertension medication today in a drip to see if it is effective so that they can get her of the N.O. and off the Vent soon.

There it is. Our precious little, silly, happy, munchkin needs a touch from God today.

I keep praying that the Holy Spirit breaths His BREATH into her which is perfect and healthy and would bring health to her lungs.

KATIE GRACE POST OP of a LARGE VSD ClOSURE +PDA REPAIR + PULMONARY HYPERTENSION - DAY 4 UPDATE

We are up and then we are down. That is how it is. I've been frustrated that every time I leave, is the time that Katie Grace Hua Hua wakes up. Apparently she is so upset and panics and the nurses have to hold her down and give her more medication. Well, I finally realized that the reason she is so quiet and doesn't move a bit for hours and hours when I am here and then has these episodes when I am not, is that although she is "sedated", she is aware of my presence.

She is still intubated and sedated now on day 4. The Drs were able to wean her oxygen down to 80% and her Nitric Oxcide to 5% and she is tolerating that. This morning however has had some very rocky times. She has been waking a lot and then gagging and then getting upset and her stats go down a bit. Well they decided to clean her tube and change the tape, but didn't want her to move around a lot so they gave her some pain med and a temporary paralytic. She was not asleep all the way though and tears rolled down her face and I knew she was terrified to be paralyzed. Then after that she was still crying and not able to move still and her heart rate went up to 160. So she had to get more medications. I am just feeling so down this morning. I feel so helpless to watch her and not be able to help how she feels and how scared she is. She may have to endure this for several more days.

PLEASE PRAY FOR KATIE GRACE! SURGERY UPDATE MORNING OF DAY 3 AND ALSO SOME PICS FROM HUA HUA'S BIRTHDAY PARTY BACK IN FEB

So as I said, I have been a little pre-occupied and have been behind on posting pics on my blog, so I figured while I am here in the hospital spending many hours in a chair, why not catch up on some pics I had hoped to post?

First, a morning update on Katie Grace Hua Hua's surgery and recovery. It is 8AM on Day 3 and she had sort of a hard night. They had noticed that her blood oxygen levels were good so they took her oxygen support level down to see how she would do. Well she did not tolerate that well so they ended up having to put her back up to 100% help on oxygen. In addition to this, she is on numerous medications, at least 8, and is still running a fever. They have her sedated and had to give her a paralytic this morning to do a procedure on her breathing tube. Although that paralytic was temporary, it made me feel sad. I feel worried and sad. I can't stand to see her so medicated and supported. I had expected to see her making more progress by now. They said they just have to take it slow. She is strong and I know she can get through this!

KATIE GRACE HEART SURGERY END OF DAY 2 POST + JOSHUA BROKE HIS WRIST!


Katie Grace Hua Hua had sort of a hard day today. She was heavily sedated earlier in the day. The Dr decided that they should "dial down" the sedation a few times today. I was meanwhile called by the school nurse and told to come pick up Joshua because he fell off the monkey bars. So I left and as I was taking him to the clinic, apparently Katie Grace woke up a little too much and PANICKED. She was still intubated. Can you imagine having that tube down your throat, not to mention waking up after heart surgery will all those lines attached to you and mom not in sight?? Well, they were able to get her sedated again, but she did cause her stats to all go in bad directions. So they had to keep an eye on her. Meanwhile, Joshua did indeed have a small fracture in his wrist.

The good news is...she is stable and sleeping. And he thought the splint and sling were "awesome"!

Pray for her lungs as they wean her tonight and tomorrow and for her to be able to come out of sedation without panicking and ruining her delicate surgery.

MORING UPDATE AND LET'S GO BACK IN TIME A BIT :) 1ST- CHINESE NEW YEAR

Hua Hua is still sedated this AM and is doing well. She is stable, still coming down slowly off breathing assistance, fever is down more w/out the help of antibiotics at 99.3.

Since I am sitting here in the hospital with not much to do but look at her, I thought I would post some pics of events that I never posted but had meant to post.

First, here are a few pics of the Chinese New Year Festival in Orlando. They had a parade the week before that I was really sorry we couldn't make, but this was businesses and restaurants with booths, various dance companies, martial arts and other performances. It was fun!

I tried to get Hua Hua to wear her authentic red Chinese dress that day, but she didn't want to. Then when she saw this beautiful girl in her dress in the show, she said, "Mama, I wear my dress now!!!" Well, I told her it was too late because we were about 35 min from home and she was pretty mad about that! :)

TODAY"S LAST UPDATE

Well Hua is still sedated and is in good care. She has been running a fever for a few hours and it got as high as 102.5 for awhile even with the Tylenol they gave her it hasn't gone down but about .5 degrees. They will give it one more hour, and if she doesn't see that fever go down more they will give her antibiotics and run some labs for infection. Keep that in your prayers.

The good news is, it does seem to be creeping down VERY slowly.

Also, more good news, they have been able to continue lowering her Nitric Oxide so her oxygen levels are doing well!

I have to go home to sleep because I cannot sleep in the room tonight and Ronald McDonald house is full. But I will be calling the nurse and be back at the crack of dawn. They will let me sleep in here when she is off the N.O.

ANOTHER UPDATE

Wow! All this stuff for one little munchkin!



She is out!


Hua Hua is in the Pediatric Cardio ICU and is doing nicely.

The Dr was able to close the hole and did not leave a hole in the patch for "pop off" as planned but created another small defect to allow "pop off pressure".

She came off bypass and heart began again well.

They closed the chest and she was ready to be moved to PICU.

She is still intubated and sedated but the Anesthesiologist said she was doing very well and was already weaning her off the Nitric Oxide by the 10s. They had originally thought she would stay on N.O. for days. In fact he said they could wean her off more quickly but she might panic and pull out her lines. So that is a good sign!



SURGERY UPDATE

The surgery nurse just came in and said the the Dr. just completed the repair on the VSD and PDA and that went successfully! Now they are going to "warm her up" and get her off the bypass machine and see how she does with her heart beating on it's own. Her breathing will remain assisted at this time.

Good morning everyone. Sorry I haven't posted much this week. Believe it or not, the day is here. The day is here, Katie Grace Hua Hua is actually in surgery this morning.

We are at Arnold Palmer Hospital in Orlando. The Pediatric Heart Team is amazing. A leading team in the nation. She is in good hands. Of course she is in God's hands so he guides the Drs hands so she couldn't be better off!

She is having a very large hole (VSD) repaired, along with a PDA (weak wall) repaired. The PFO (another hole) is being left alone. The main concern is her Severe Pulmonary Hypertension. This should have been taken care of a couple years ago but that was not in the plan for her. Now is her chance.

We were told this morning that they will probably have to keep her sedated and intubated for a few days after surgery due to her lungs. I wan't planning on that, but I know God has the plan in his hands. Pray for her lungs to have an amazing recovery!

Happy Birthday To Me! Some fun things about me. :)

I celebrated my Birthday last Saturday. Let's say I am over 40. :)

I love Birthdays. I am not one of those people who says, "oh don't worry about it" or "let's not do anything". I love to celebrate Birthdays! It doesn't have to be a huge party, but there does HAVE TO be CAKE.

I especially love CAKE.

CAKE, CAKE, AND MORE CAKE. I like gooooood cakes. Good frosting. I just savor it all week. Not the cheap frosting that some grocery store cakes have that sticks to the roof of your mouth like grease. I mean real, sweet, good frosting.

Having a piece of cake with coffee and being grateful for another year of life.

My sister-in-law visited on Saturday and she brought a cake that my mother-in-law sent from Mobile from Cakes By Judy. My favorite!! So yummy!

Yes, I really can talk this much about CAKE!

Birthdays are that one day to celebrate YOU! It's nice to have one day to say, it's fun to be ME!

Anyway, I am so thankful for my Birthday. I am thankful for another year to enjoy with my family. My husband. My kids. My friends. My job.

I don't talk too much about my job here on the blog, but I want to say that I love my job. It brings me so much joy. I have worked for the Disney Co. for 22 years. 22 years! Wow, that has flown by! I am a singer, comedian, dancer, actor, host, storyteller and more. I laugh with my friends everyday! How fun is that?

Well..Surgery has been postponed once again...


Due to the cough that Hua Hua had, but doesn't have anymore, but did have, although not a trace of it is left, but hasn't been over long enough, ahhh, well, you get what I'm saying. They are not doing the surgery this week.

They have postponed Katie Grace Hua Hua's heart surgery until next week on the 22nd.

Don't get me wrong, I totally understand that they have to be very careful, and I want her to be safe above all! It is just hard to get all worked up and planned for a surgery and then have it cancelled and rescheduled at the last minute several times. Not to mention taking off of work, losing all of those hours (when you are an hourly worker and not exactly rolling in the dough :) ), then having to take off more work, and also changing families' plans who are trying to come from out of town.

But we will get through this and our little girl will have her surgery and most importantly God will bring her to health!

So don't get too set on next week, but pray that it will happen for sure!!! Let's believe to to be so!! Thank you for your prayers my dear friends!!

SURGERY HAS BEEN RESCHEDULED

I know I am not a good blogger these days. I hope to be a good one again soon.

Katie Grace was originally scheduled to have surgery this week and it has been rescheduled for March 14th. That is due to the fact that there were some other infants born into the Heart unit that have immediate needs.

Unfortunately, she has been very sick this week as well. Fever for 4 days and a cough, so it is possible they may even add one more week to that date, but I will keep you all posted.

Thank you SO much to you dear friends who have emailed me to check in and see how she is doing!
